Hospital visits can be an incredibly confusing and upsetting experience for someone with dementia, and even if you want to be by their side all the time it’s often just not practical. So you need to know that the people caring for your loved one are aware of their needs and preferences when you can’t be there.

Zoe Harris designed this Pocket Care Chart as a result of her late husband’s experience, who endured a number of hospital stays when his dementia was already fairly advanced. Because it features the same symbols as the Wall Care Charts it is quick and easy to transfer the information in the event of a hospital visit. Small enough to slip in a pocket or handbag, it’s still got enough space to record an individual’s needs and preferences, and it’s printed on robust card so that it can stand on the bedside table.
